Subjetividades discursivas del dolor crónico femenino: una aproximación psicoanalítica

Las estadísticas recientes demuestran que se reportan una mayor prevalencia de dolores crónicos y fibromialgia en mujeres. La revisión de literatura sugiere que la mayor parte de los abordajes contemporáneos atienden el dolor crónico femenino desde perspectivas que se enfocan en aspectos cognitivo-conductuales, biológicos y hereditarios. El propósito de esta investigación es explorar las temáticas femeninas de los malestares subjetivos con relación al dolor crónico mediante un enfoque cualitativo y de alcance inductivo-exploratorio. El análisis discursivo lacaniano utilizado como método de análisis permitió tomar en cuenta significantes y metáforas para acercarnos a elementos discursivos de la dimensión subjetiva del dolor crónico femenino y la fibromialgia. La fuente de información para el análisis se obtuvo través de blogs escritos por sujetos femeninos con dolores crónicos. El análisis utilizó como referente la teoría psicoanalítica, particularmente los trabajos de Sigmund Freud y Jaques Lacan. A partir del análisis de los hallazgos, se identificaron ocho temáticas discursivas: los alcances de la nominación, la función del dolor, el dolor y la repetición, el dolor y los límites, el dolor y la cuestión del deseo, el dolor y las marcas del Otro, el dolor y el duelo, y el dolor, el amor y lo femenino. Se plantean las siguientes coordenadas teóricas para pensar el dolor crónico femenino: la distinción teórica entre histeria y feminidad, histeria (síntoma y estructura), la feminidad y lo femenino, y el duelo. Desde la ética del psicoanálisis, se reitera lo fundamental de una escucha más allá del diagnóstico y el cuerpo biológico. Así también, se propone un espacio clínico que invite al sujeto a un encuentro con la palabra, con su memoria y con su historia, creando así las condiciones para que pueda interrogar la posición que asume ante su sufrimiento y su dolor.
